Intermediate Care
Intermediate care is a person-centred care approach using short-term interventions to maximise independence. Its success relies on multi-agency and multidisciplinary working to create shared assessments and joined-up plans of care. This one day course will enable staff to further understand each others roles in the team and how to enable clients to utilise the resources available to them.
